In several medical centers throughout the community, we now offer Medicaid eligibility and re-certification options for individuals and their families. Each of the following medical centers has full time staff members dedicated to helping patients apply for eligibility for benefits through Medicaid or re-certify their eligibility if needed.

Medicaid Access Eligibility & Re-Certification  
Florida Medicaid is the medical assistance program that provides access to health care for low-income families and individuals and also assists aged and disabled people with the costs of nursing facility care and other medical expenses. Eligibility for Medicaid is usually based on the family’s or individual's income and assets.

Continucare understands that having the knowledge and understanding of the eligibility requirements can be the difference between receiving or not receiving the medical care you or your family needs. We have added new Medicaid eligibility and re-certification stations at several of our medical centers in your community. We invite you to learn more about Medicaid and to visit one of these medical centers to find your eligibility status and options.

  Who is Eligible for Medicaid?
An individual must meet specific eligibility requirements in order to get Medicaid and based on those requirements, individuals may be eligible for full or limited benefits, depending on the program. Medicaid programs are designed to provide benefits to three basic groups:
  • Supplemental Security Income (SSI) recipients
  • Children and families
  • Aged, blind and disabled people, including people needing institutional care
 
 
  What Benefits Are Available Through Medicaid?
For each group stated above, there are varying levels of limited and full benefits that are covered with each plan. Once eligibility is determined, your Medicaid Access representative will provide details on what benefits you or your family will receive. They will then walk you through the enrollment process so you or your family can begin to receive needed medical care.

 
 
  Where Can I Check My Eligibility?
Continucare now has added eligibility and re-certification access at several of our medical centers in your community. There are full time Medicaid application specialists staffed at each of these locations to assist you in applying for or re-certifying your enrollment to receive Medicaid benefits.
